Welcome to the new patient center!

Arriving at a doctor's office for the first time can be a nerve racking experience. Paperwork to fill out, new faces and names to learn, and getting to know your way around the office; this can hit the senses all the at same time causing overload.

In contrast, at our office, our aim is to remove any stressor causing unnessesary tension and make you feel comfortable. In this area of the website you will be able to take care of items necessary for your visit prior to arriving, in the comfort of your home. These items include:

• Health History Form
• Financial Responsibility Form
• HIPPA Form
• Authorization to Treat a Minor Child Form

THESE FORMS ARE AVAILABLE FOR DOWNLOAD UNDER OUR NEW PATIENT CENTER/ONLINE FORMS.

Information (if applicable) to bring with you to your first appointment:

1. List of previous physicians seen.

2. List of medications and vitamins/supplements you are currently taking.

3. Laboratory results

4. X-ray, CT scans, or MRI scans preferably on CD with reports

5. Specialists' reports

6. Insurance information (while we are out of network for all insurance, including Medicare, we will file with your insurance company on your behalf with any reimbursement sent directly to you).
