Your First Visit

Patient Forms

Upon entering our office, you will be asked to fill out our new patient forms, including medical history (contact information, current medications, present symptoms, previous physicians seen, previous surgeries), HIPPA forms (patient privacy), ABN (advance beneficary notice), and financial consent forms.  For patients under the age of 18, an authorization form must also be completed by a parent or guardian.  This paperwork provides us with general information about you and your overall medical condition.  To save time on the day of your initial visit, these forms can be sent to you ahead of time upon request via fax, email, or regular mail.  You may also pick up them up at the office or download them directly from our website.  The completed forms may then be faxed to our office prior to your first visit or brought with you.  TO DOWNLOAD A COPY OF THESE FORMS, GO TO THE NEW PATIENT CENTER/ONLINE FORMS SECTION OF OUR WEBSITE.


Insurance and Medicare

Dr. Frasure believes in the value of wellness care for you and your family and has maintained a pricing structure that allows care for all budgets. While we will file all insurance paperwork for you, we are out-of-network for all insurance.  This means that you are responsible for payment at the time of your visit, and any insurance reimbursement from your insurance company will be sent directly to you.  Check with your insurance company if you have questions about out-of-network coverage and benefits, as plans and coverage options vary widely among providers.  For patients with special circumstances, flexible payment programs can be arranged.  To learn more about our payment options for chiropractic and wellness care, you can call our office directly.  Our staff will be happy to help you understand your insurance coverage and benefits. 

If your insurance provider does directly reimburse you for chiropractic care, be sure to bring your most current insurance card with you on your first visit.  We will record your policy information and file the insurance claims for you to expedite your receiving reimbursement. 

We are also out-of-network for Medicare, which also means you are responsible for payment at the time of your visit.  If you have insurance coverage in addition to Medicare, it is important for you to know whether this is supplemental or secondary insurance.  Supplemental insurance does not pay you unless Medicare pays first.  Secondary insurance, on the other hand, may pay even if Medicare does not pay.  Check with your insurance provider to clarify whether your additional policy is supplemental or secondary and if you have chiropractic benefits.  If secondary, be sure to bring your most current insurance card with you on your first visit, so we can record your policy information and file the insurance claim for you.  If you are not sure, our office can assist you with this information.

Many patients pay directly for care, as they discover chiropractic to be extremely cost-effective and affordable.


Consultation and Examination

Next you will have a consultation with Dr. Frasure to discuss your health-related problems, concerns. Your first visit is designed for Dr.Frasure to learn more about you, your condition and your expectations to determine whether chiropractic care will you meet your goals.

After your consultation, Dr. Frasure will perform a complete chiropractic examination testing your reflexes, your ability to turn and bend as well as other standard neurologic, orthopedic, postural, and physical examinations.


X-Ray Studies

While we do not take x-rays in the office, if your specific condition requires x-rays to be taken, we can write an order for x-rays.  Due to various health plan restrictions, we may have to order x-rays from a specific location or go through your primary care physician.


Report of Findings

Once all the information and examinations have been performed, Dr. David Frasure will discuss his assessment, his recommendations, and his plan for you.  If you have a condition that requires care with other providers, the doctor will tell you and make the referral. If Dr.Frasure believes we can help you, we will recommend treatment and a wellness program.


Treatment

Patients will generally be provided with their first treatment at the conclusion of the initial appointment. This may include spinal adjustments, physicaltherapies, and/or soft tissue therapies.  Treatment is interactive with you so you can express any concerns you have about the style of adjustment you will be most comfortable with. Dr.Frasure has been treated in various adjustment techniques and will tailor them to your needs.

The following is a list of techniques he has taken:

Diversified, Gonstead, Cranio-sacral, SOT blocking, NET (Neuro Emotional technique), Applied Kinesiology, Arthrostim, Activator and Brimhall technique

The following is a list of physical therapy modalities available:

ElectroMyoStimulation, Therapuetic Ultrasound, Percussor, Near infrared Low Level Laser Device, Color and Sound.


Wellness Program

Prior to leaving, patients will be instructed on a wellness program to be conducted at home. If you are in pain when you first come intor our office this may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches.  If you desire an overall wellness lifestyle, we will work with you to create good habits and routines that support your success.  Every body is unique and different and therefore everyone requires a unique and customized wellness plan.  The foundation of our overall wellness lifestyle is maintaining  healthy posture and good spinal alignment, eating a healthy diet, taking supplements, keeping your weight under control and stress relief.
